Das unknown ist bei einem Kommentar impliziert, sodass `"Termin nach Vereinbarung / on appointment"` und `unknown "Termin nach Vereinbarung / on appointment"` das äquivalent sind. Wichtig sind nur die Anführungszeichen, da es damit nach OH-Syntax ein erklärender Kommentar wird.
Ich würde aber definitv Deutsch wie Englisch setzen, da ein Deutschland Deutsch gesprochen wird und Englisch für den Rest von Bedeutung sind könnte.
Wenn OsmAnd das falsch auswertet, sollte man dort ein Issue einreichen.
Langfristig ist aber der Öffnungszustand `appointment` für die Syntax wichtig für die Auswertbarkeit und damit unter anderem auch Übersetzbarkeit.

appointment ist kein korrekter OH-Wert, leider.
Es gab zwar mal osm.wiki/Proposed_features/Opening_hours:_standard_appointment_syntax. Daraus wurde (bisher?) aber nichts.

Man könnte hier evtl.
opening_hours=unknown "Termin nach Vereinbarung / on appointment"
setzt.

But the article mentions the current name (same for the wikidata item) and the renovation plans, so that article will likely continue to be relevant for this hotel.
